I think it's second. It used to not be visible on GS, because there were simply a few very populated instances. If you had your own single-user instance that GS behaviour could make you mad, because you were sometimes missing 70% of conversation, when people unknown to your instance (not followed by you) were replying to someone (followed by you) taking part in same conversation as you do. One of the main features listed on Mastodon's GitHub is actually a fix for that problem.

Now, with 1000+ Mastodon instances, many of them somehow connected to GS, that problem (or feature, how some people call it) is just much more visible. -

You can have an absolute saint for a CEO but at the end of the day the system they find themselves in legally mandates that they exploit others as much as possible.

Everyone has a "this has become a stream of nonsense" threshold. The silos try to get around it with algorithmic timelines, but that only adds other problems. -

It has been said before that there may be an optimal community size, and something like the Robin Dunbar observation of 100-150 seems common. Beyond that it's mostly just entropy or you run up to the physical limits of how much you can read and write in a given time span.
If you're designing for federation or self-hosting then maybe optimizing around this sort of number is reasonable, and I expect that it happens already anyway but without explicit knowledge. -

Anything which grows too much will fail as a community. However, as this is all !fs the important thing to remember is that these are mere tools to create our own, new and balanced communities.
I for one have seen many Twitter users come and go in floods to the !fediverse, some staying to create instances of #GNUsocial I think are despicable and don't agree with. But they are only a problem if you lack moderation.
Instead of "web-scale" we should think "human-scale".
